The Health Information Management department maintains records of the care patients receive while they are at Temecula Valley Hospital. As required by state and federal law, these records are kept in strict confidence and are only released with proper authorization from the patient. The Centralized Release of Information department is located at 25485 Medical Center Drive, Suite 208, Murrieta, CA 92562. The department is open from 8:30 a.m. to 4:30 p.m., Monday through Friday.

Radiology images and studies require one full business day to prepare. If you are scheduling follow-up appointments and will need these materials, please plan accordingly.
After we have processed your request for medical records, we will mail the information to the address specified on your request form, or you may pick up your copy from our office. Due to the large volume of requests we receive, it may take up to 5 business days to fulfill your request from the date we receive it.
Please bring your insurance card and form of payment to each visit. We accept Visa, Mastercard, cash and checks.
We accept most insurance plans. For patients without insurance, we charge $89 per visit. We charge $130 for physicals, which include an EKG, vision and hearing screens and urinalysis. Charges may be higher for additional services.
