How do I login to the patient portal?

In order to access the patient portal, you have to be linked by your Doctor's office. Please use your complete email address on file with the office for your User ID. Please refer to the three welcome emails you received once linked for your temporary password. If you have not been linked to the portal, please contact the office.

How do I self-enroll in the mytmh patient portal?

Patient Forms

If you do not wish to use the NextGen patient portal, you can download the patient forms here. Either print the form, complete it, and bring the hard copy with you to your appointment; or save a digital copy on your computer, fill it out, and upload the completed file online.

The instructions below detail some of the procedures we perform at Tallahassee, Ear, Nose and Throat. Our pre-operative instructions help patients prepare for their specific procedure. Sometimes our patients will also need to follow discharge instructions afterward to help them feel better and recover fully.

New Patients

If this is your first visit with our office, or if you have not been seen by us in the last three years, you will be asked to complete the “New Patient” registration process. Your new patient paperwork can be completed electronically using our secure patient portal which saves time when you check in.

Telehealth

Virtual visits with your doctor or audiologist may be possible for your convenience when deemed medically appropriate by your provider. To learn more about this service, please click the button below.

Online Paperwork

When scheduling your appointment, our staff will let you know which forms are necessary for you to complete before your telehealth visit. You may need to complete new patient paperwork and/or update insurance cards/IDs on file. All documents and scans must be sent prior to the start of your scheduled appointment.

Online Payment

Our staff will tell you the estimated cost of your telehealth visit when you make your appointment. Payment is due prior to the scheduled time of your telehealth service, and you may make your payment via our online billing system.

What is a patient portal?

The Patient Portal is a secure, online service center where you can access your personal health information, address administrative needs, and make non-urgent medical requests. And having access to your information will also help you stay informed and in charge of your own care.

How long does it take for the Patient Portal to close?

To help protect your personal health information from unauthorized access, the Patient Portal has been designed to automatically close after 30 minutes. It’s recommended that you save periodically to avoid any information from being lost should the system time out.

Does the Portal upgrade affect existing accounts?

The upgrade will not affect existing accounts, and the Portal will not be taken off-line. It will, however, affect access through outdated/obsolete Operating Systems and Browsers. Please plan to upgrade your systems if required.
