The MEDENT Patient Portal provides you with secure, online access to portions of your medical records, as well as an easy and convenient way to communicate with the practice through a secure internet connection. Think of it as an online account for your doctor’s office, similar to online banking or credit card accounts you may currently have.
New patients can request intake appointments and complete new patient forms in the patient portal. Existing patients can manage appointments, view documents, and update preferences using our patient portal. Synergy of Monticello Inc. has a calling system that calls patients 3 days before his or her appointment as a reminder call, in which the patient can confirm his or her appointment or reschedule it.
If a patient “No Shows” for an appointment, the patient will be billed for the full visit for the missed office visit. This is not a billable charge to the patient’s insurance company; it is the personal responsibility of the patient.

The patient portal gives you access to your medical records and lets you securely communicate with our office. When you sign up, you will receive an email with instructions for logging in.
We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office, and on our web site.

To activate your account, you will need to receive an activation code from us. Once you have received the code, you will be able to create your own username, password, and other login information used to verify your identity.
