How to access patient portal?

Once you have set up your email address and password, you will be able to access the Patient Portal. To begin, click on the Patient Portal link on this page. You will be directed to a secure login page prompting you to enter your registered email address and password.

How to make a payment on a patient portal?

Once logged in, you have the ability to make a payment. To begin, click Bill Pay. Next, choose the amount for payment. Finally, complete the Payment Options screen to enter your payment information. Once payment is complete, the patient portal automatically signs off your online session after a period of inactivity to prevent unauthorized use of your account and prevent others from viewing your information.

How to get a paper copy of my medical records?

If you would like to request a paper copy of your health information, please click on the link Request My Medical Records on this page. A document will download to your computer for you to print and complete. Once completed and signed, you can either mail or fax the form to the address/fax number in the top right-hand corner of the form. As an alternative, you may bring the completed form to Surgeons Choice Medical Center. Once arrived, please ask security for Medical Records and someone will greet you at the security desk.
