How to request a medical exemption?

To request a medical exemption, you must submit signed and dated documentation from a licensed physician, physician’s assistant, or nurse practitioner. This document must contain a description of why vaccination is contraindicated.

Where is SHS located?

Student Health Services (SHS) is located in Laurel Hall, a freestanding ambulatory care center on the main campus south Green area at the intersection of South College Avenue and East Park Place. This well-equipped facility provides medical treatment and office consultation space for a variety of outpatient services.

When is UD surveillance testing available?

UD surveillance testing is available Monday through Thursday by invitation and appointment only from 9 a.m. to 1 p.m. at Harker ISE Lab and 10 a.m. to 2 p.m. at Clayton Hall. What forms are required for 2021?

All incoming students are required to submit two completed forms: the Immunization Record and Medical History Form. Beginning spring 2021, both forms can be completed and submitted electronically. Failure to complete either form will result in a hold preventing class registration for future semesters.

Emory University Student Health Services (EUSHS) provides outpatient care for enrolled Emory students with a valid Emory ID card. International student's spouses, Domestic Partners and unmarried children over 18 years of age are also eligible for primary medical care if they are currently enrolled in the Emory/Aetna Student Health Insurance Plan.
