We are writing as a follow-up on the Provost’s email from July 16 regarding the UC COVID-19 Vaccine Program. This policy requires all UC students to receive a COVID-19 vaccine in order to physically be present on campus for attending classes, for living on campus (in UCI or in ACC housing) or for working on campus beginning this fall.

Vaccine Verification via the Student Health Center Portal

The UC policy requires individuals to show proof of vaccination. Once you receive your COVID-19 vaccine, please upload your vaccination record on the UCI Student Health Center Patient Portal. There are step-by-step instructions for uploading your vaccination record. In addition, this short video tutorial walks you through the upload process.

Vaccine Assistance

Any student who needs assistance obtaining a COVID-19 vaccine and/or uploading their vaccination record to the appropriate portal may contact Contact Tracing and Vaccine Navigation Services (CTVNS). They can be reached at 949-824-2300 or contacttracing@uci.edu.

Vaccine Exemption, Exception or Deferral

Instructions regarding how to request a medical, religious, disability or pregnancy exemption, exception or deferral and the applicable request forms can be found on the SHC website. All request forms must be uploaded to the UCI Student Health Center Patient Portal. Please note that the review of your request may take up to seven (7) business days.

Vaccine Availability in Fall

The COVID-19 vaccine will be available through on-campus Student Health Center Vaccine Fairs for Fall Quarter, along with ALL other required vaccines. HOWEVER, UCI students SHOULD NOT plan to wait for these events to ensure they are compliant with the policy.

Information for International Students

For students who have received a COVID-19 vaccination outside of the United States, UCI will accept vaccines authorized by the World Health Organization (WHO). To find more information on authorized vaccines on the Emergency Use Listing/Prequalification (EUL/PQ) list, visit the WHO website.

Vaccination Non-Compliance

Students who do not comply with UC vaccination requirements, including the COVID-19 vaccine mandate, may be subject to disciplinary action or loss of campus privileges, including but not limited to loss of wifi privileges, restriction from campus resources, restriction of access to campus, and an academic registration hold restricting students from registering for classes.

Can you enter the SHS building for dental appointments?

Remember that you must complete the Daily Wellness Check before coming on to campus. You will not be allowed to enter the SHS building for medical and dental appointments or pharmacy pick-up or drop-offs without clearance.

Confidentiality

  • All patient information is confidential and protected by privacy laws. Information from your records will not be released to anyone without your written authorization, except as required by law. Our Notice of Privacy Practices describes how medical information about you may be used and your rights as a patient.

Retention of Medical Records

  • The Student Health Center retains patient medical records for a minimum of ten (10) years from the date of last visit. Active records are on file at the Student Health Center. Records of patients whose most recent visit is more than five (5) years from the current year are maintained in an off-site storage facility.

Requesting Copies of Medical Records

  • In compliance with patient confidentiality and privacy laws, health information may not be released without a patient’s written authorization. To request medical records, you must submit an Authorization for Release of Health Informationonline via the SHC patient portal; in person; by fax; e-mail; or by mail. Charges For Medical Record Services

  1. Copies of medical records will be sent to another healthcare provider free of charge.
  2. A fee of $30 will be charged for requests from attorneys, insurance carriers or other third parties.
