If you have an HMO insurance plan, please obtain from your insurance provider, prior to your appointment, a written authorization to be seen at SHC. If your insurance benefits do not cover your visit, you will be asked to sign a Patient Acceptance of Financial Responsibility form upon arrival and submit payment upon receipt of your bill.
