If you are not a St. Peter’s Health patient but want electronic access to another adult’s health care information, you must complete the Patient Portal Proxy Registration form. A non-St. Peter's Health portal will be created for you.
St. Peter's Health Partners is transitioning to a new patient portal – Trinity Health MyChart – in May 2021. After the transition, any new information, including prescription refills, visit notes and lab results, will be in your new MyChart patient portal.

Your Medical Record Number starts with an “M” and looks like M000055555. The entire number (including zeroes) needs to be entered when using it to sign up for Patient Portal on-line. Access Historical Health Information. You can still access your historical medical information from our previous patient portals (athenahealth and MyHealth). However, you will only be able to view and read the information; you will not be able to communicate with your provider and no new information will be added.
