Via MyChart, you can communicate with your doctor, access test results, request prescription refills, and manage your appointments. We offer MyChart to patients at the following hospitals:

Brazosport Hospital Patient Portal

For patients at St. Luke’s Health–Brazosport Hospital, we offer a separate, secure portal that allows you to access your health information, update your personal information, check account balances, and view scheduled appointments all in one place.

Patients Medical Center Patient Portal (MEDITECH)

For patients at St. Luke’s Health–Patients Medical Center, we offer access to MEDITECH. Log in to your account to find your medical records, schedule a visit, and more.

Memorial Hospitals Patient Portal (MY ONECARE)

For patients at St. Luke’s Health–Memorial, we offer access to MY ONECARE. Log in to your account to find your medical records, schedule a visit, and more.
