Receive a copy of your, or someone else’s, medical record. There are two ways to request a copy of someone’s medical records, online and by mail. You will have the option to select what format you would like to receive the records (PDF, compact disc, or hard copy). Processing your request may take up to 30 days.
Proxy access is when a family member or caregiver can view a portion of a patient’s medical record including protected health information (PHI). The access must be authorized by the patient, and multiple parties may be given access. A proxy is not the same thing as a guarantor.
