Per State and Federal guidelines, after the child reaches age 12, parents/guardians will need an additional authorization signed by the child to have access to the child’s medical record information. Once a child turns 18, a parent can only access the child’s medical record information with the child’s authorization.
In addition to handling your own care, parents and healthcare proxies with authorized consent will appreciate the ability to access the records of those individuals for whom they manage care.

Rest assured your information will always be kept confidential between you and your Provider. However, you can choose to give another person access to your Ascension St. John Patient Portal account. They will be called your proxy. If you are in person at your clinic, you can fill out the required paperwork to add a proxy. If you are registering online, please call your clinic to add a proxy.
