Michigan Medicine has also developed a program to provide heart failure patients with all the resources they need for success and to minimize the chance for readmission.

How to create a follow my health account?

Registering and creating a FollowMyHealth account is easy, just follow these simple steps: 1 Download and print the FollowMyHealth form ( Proxy form) 2 Follow the instructions on the form, filling it out completely 3 Don’t forget to sign and date it at the bottom 4 Return the completed form to your physician 5 For access to hospital visits, please bring form to the hospital’s Registration department to be forwarded to the appropriate personnel 6 ID is required when you submit your form 7 Once you have submitted the patient user agreement form and a healthcare provider has entered your information, you will receive an email from noreply@followmyhealth.com with a registration link. Click on that link. 8 You will be asked to enter an Invite Code to verify your identity. This is your 4-digit birth year. 9 Create your portal login with a unique username and password. 10 Enter the invitation code provided to you by your healthcare provider and click "Agree” to the release of information. 11 You will receive two emails: one welcoming you to FollowMyHealth and one confirming your connections with St. Mary’s of Michigan or St. Joseph Health System. You will then have complete and secure access to FollowMyHealth and your personal health information.
