MHSC's patient portals are not available at this time. The hospital is in the process of a major technology upgrade. It soon will offer a new, single electronic health record to better serve our patients' needs. By March, your hospital and the Speciality Clinic health information will be available through one portal.
Patient Portal allows patients to view the central appointment page which displays their list of scheduled and pending appointments. Here, they can review appointment details. Patient Portal also saves patients time by allowing them to submit updates to their demographic and contact information prior to their visits.
Care management at Memorial Hospital Central is here for you. We work with you, the patient, and your family as an advocate during your hospital stay. Our team consists of Care Managers, Social Workers, and Care Coordinators. Our goal is to work with you, your support person and your health care team in developing a discharge plan so your ongoing care needs can be met when you leave the hospital. We strive to maintain community partnerships for the health and well-being of our patients.
Being in the hospital, or having a loved one hospitalized, can be stressful and emotional. Chaplains are available to help you work through the anxiety that often accompanies a hospital stay and talk with you about how hospitalization affects your spiritual journey.

This interactive web portal will empower patients and their families to take a more active role in their care by providing easy, secure access to their health information and online communication with Clinic staff.
