What is a patient portal registration form?

An individual Patient Portal Registration Form is a legal document completed by by an adult patient giving St. Peter's Health permission to create a Patient Portal.

How to reset patient portal password?

Your log on ID and password can be reset by using the links on the Patient Portal logon page. This will also unlock a locked Patient Portal. When these requests are made the information entered must match your Patient Portal account. If you need assistance, please contact Patient Portal Support.

How old do you have to be to sign up for a patient portal?

Please note, you must be 18 years and older or emancipated to sign up for a Patient Portal.

How to find medical record number?

Keeping your health care information private and secure is of the utmost importance. Medical Record Numbers will not be provided by email or over the phone. Your Medical Record Number starts with an “M” and looks like M000055555. The entire number (including zeroes) needs to be entered when using it to sign up for Patient Portal on-line. You can find your Medical Record Number on: 1 Your billing statement 2 Your hospital discharge papers 3 Your hospital wristband 4 Your clinic visit summary

How to register for a hospital?

To register for MyHealth during or following your hospital visit, simply: 1 Visit the web site: sphp.com/myhealth 2 Click on “Enroll or Log Into MyHealth” 3 Select “Click Here for Patient Signup” 4 Verify your identity with your email or text-capable cell phone number 5 Establish and maintain a secure User ID and Password for accessing your online MyHealth account

Once your MyHealth account is set up, requests for assistance can be submitted online or you can contact the MyHealth Help Line at 518-525-1771, Monday through Wednesday, 8:30 a.m. to 4 p.m.
