Want an invitation?

Connecting to your NIH Clinical Center medical record is a two-step process. The NIH Clinical Center FollowMyHealth ® Patient Portal requires an invitation from our organization in order for you to see your NIH Clinical Center medical records. If you have not received an invitation you can request a connection to our organization.

How to set up a patient portal?

Here’s how to set up your patient portal account: 1 Verify your email address when you check-in for your appointment so that after you visit HFSC, you will be sent an email invitation. 2 If you would like to request an invitation to the portal, contact the Health Information Management Department by sending an email to ROI@hfsc.org or by calling 860-827-4863. 3 If you would like to request a patient portal invitation for an Authorized Individual who will access your account for you, please select the type of authorization form you need:#N#Authorized Individual: for adults who wish to authorize a caregiver or family member to access and administer their account, or when a legally appointed representative (such as a conservator or guardian) needs to access and administer the patient’s account on his/her behalf.#N#Authorized Parent/Guardian: for children (under age 18) whose parent or guardian has the authority to access and administer their account.

Who is an authorized individual?

Authorized Individual: for adults who wish to authorize a caregiver or family member to access and administer their account, or when a legally appointed representative (such as a conservator or guardian) needs to access and administer the patient’s account on his/her behalf.

What is the 21st Century Cures Act?

This is in response to the 21st Century Cures Act which ensures patients have timely access to their electronic medical records via the patient portal. You may see results or care findings before your healthcare provider has been able to review them, so please allow time for your healthcare provider to review the results.

What is FollowMyHealth?

FollowMyHealth® is used by healthcare organizations across the country as the power behind their hospital or clinic’s specific patient portal. Log in with your username and password to access your information.

Is FollowMyHealth a complete medical record?

FollowMy Health® is not your complete medical record. FollowMyHealth® provides you with access to some (but not all) of the information contained in your electronic medical record. If you would like to request a copy of your complete medical record, contact Health Information Management department at ROI@hfsc.org.

TOL Secure Patient Portal

The TOL Patient Portal (also referred to as "TRICARE Online" or "TOL") is the current secure patient portal that gives registered users access to online health care information and services at military hospitals and clinics.

MHS GENESIS Patient Portal

MHS GENESIS is the new secure patient portal for TRICARE. It will eventually deploy to all military medical and dental facilities worldwide and replace the TOL Patient Portal.

Using MHS GENESIS and TOL Together

If you’re already a registered user on the TOL Secure Patient Portal, MHS GENESIS works much the same way.
