To confirm your mySpeare Patient Portal User ID, you need to come to the Health Information Management (Medical Records) at Speare Memorial Hospital. We are open from Mon-Fri from 8:00am-4:30pm. You will be required to present a photo id.

Located on the second floor of the Speare Memorial at Boulder Point medical office building, just off Tenney Mountain Highway in Plymouth, the Speare Primary Care Team offers timely, efficient, and coordinated care across each patient’s individual health care spectrum and expanded care options to patients of all ages.

Effective June 30, 2021. Following the State of New Hampshire’s guidelines, Speare Memorial Hospital is relaxing some of our prior restrictions. (Visiting hours remain between 10am and 5pm.) All visitors must pass screening on entry to the facility.
Each patient may have one consistent support person who can visit them during their stay.
This means that by law, Speare cannot release COVID-19, or any additional health information, about our patients and staff.
Each patient may have that same visitor when they are in recovery and preparing for discharge.
Speare Memorial Hospital is pleased to be able to lift restrictions on our services due to the low COVID-19 transmission rate within our community .
As this pandemic evolves, the science and recommendations will change. We will continue to follow CDC guidelines on masking. Our current COVID-19 mandatory mask-wearing protocol will remain in effect at Speare Memorial Hospital and all of our affiliated physician practices for the safety of our patients, staff, and the community at large.
Setting up to attend a telehealth visit 1 Go to the App Store (for iPhone/iPad) or Google Play (for Android) and download the StarLeaf app. 2 Once an appointment has been made, you will receive an e-mail from our staff. Simply select ‘ Join this StarLeaf Meeting ,’ and you will be taken to StarLeaf’s launch. The receptionists or medical assistants will be in touch with you prior to your visit to ensure connectivity. 3 You are welcome to test the app before you attend a telehealth visit.
Testing the telehealth app or program. Before you attend a telehealth visit, it is extremely important to test the app or program where you will be at the time of the visit, such as your home or office.
On the day of your telehealth visit. On the day of your telehealth visit, you will receive a call from the receptionist approximately 15 minutes before the appointment. They will review your demographics and insurance and collect any copayments due.
