What can I do in the patient portal?

Our Patient Portal allows you to make requests (for medical records, prescription refills, and specialist referrals), correspond with your doctor (send them a message and book an appointment), view your lab results, update your personal information, and view a summary of your visits.

What is the patient portal for Family Practice Center?

FPC Patient Portal The Family Practice Center, PC, Patient Portal provides secure access to portions of your medical record and allows easy and convenient communication with our practice. So that we can provide this service without compromising your private information, no one will have access to your personal portal account except you.

What happened to St Peter's Health Partners patient portal?

St. Peter's Health Partners is transitioning to a new patient portal – Trinity Health MyChart – in May 2021. After the transition, any new information, including prescription refills, visit notes and lab results, will be in your new MyChart patient portal.

Can I still access my medical information from previous patient portals?

You can still access your historical medical information from our previous patient portals (athenahealth and MyHealth). However, you will only be able to view and read the information; you will not be able to communicate with your provider and no new information will be added. Need Help? How old do you have to be to communicate with FPC?

Parents/guardians may communicate with FPC providers and staff as you always have until your child reaches age 18.

Can a parent/guardian be a portal authorized representative?

For this reason, we find it necessary to ask our patients in this age group to complete a consent form naming a parent/guardian as their portal authorized representative. Without this consent from the minor patient, we will not be able to give parents/guardians access to the minor's portal. (Click the link below to access the form.)

Signing Up

Your doctor's office will provide you with a username and password to give you Patient Portal access. You can also get access by calling our Customer Service Department at (614) 423-6916 or toll-free at (844) 245-5538.

Telehealth Visits

In an effort to prevent the spread of disease, we are working to offer virtual Telehealth visits instead of in-person visits for certain situations.

What to do if you have problems with portal?

If you are experiencing problems with the portal, please call your provider’s office.

How to send health information to another practice?

You are able to send health information you choose to another practice by choosing the “Chart” button in Menu, selecting the information you would like to send, then at the bottom of the page selecting the “Send to Another Practice” button.

How to find an appointment form?

You can find a Request an Appointment form using the Appointments tab in Menu. Once the form is completed, a member of your provider’s office will reach out to confirm. This is for non-urgent issues only.

Can you take medication through the patient portal?

Yes, through the medications option in the patient portal menu bar.

Can you activate portals via email?

Portal activations are not sent via email, only printed letter handed to you at your office visit, over the phone or by a mailed letter. If you have not received an activation letter by any of these means, please contact your office.

Can an adult patient have a proxy?

Yes, an adult patient may designate another individual to have proxy access to their portal account. This can be done in person at Patient Registration.
