What is the patient portal?

Jun 24, 2019 · If you are an established patient at Southwest Care, you can sign up for the patient portal by clicking here. You will need to enter your first name, last name, date of birth, gender, phone number and email address. The Patient Portal will send you a temporary passcode. Select how this passcode should be sent: email, call, or text message.
