Download your patient forms online through the patient portal When you come to our office for the first time as a new patient, we'll ask you to complete some initial forms, including an Authorization and Consent for Treatment form, if you were not able to download them from the patient portal in advance of your appointment.

The kiosk is located on the first floor in the main lobby. Southside Bistro is open Monday - Friday, 7:30 am - 11 pm.
The hospital is a smoke and tobacco-free facility. The use of tobacco products is not permitted on hospital property or adjacent properties. Smoking cessation classes are offered to patients, employees and the public. Patients can discuss tobacco-free alternatives with their doctor.
However, such designated support person may be required to comply with all reasonable requirements of our facility, and be restricted to specific areas of the premises, to protect health and safety. Our facility may request documentation indicating that a patient has a disability or perform an objective assessment if no documentation is provided.

Dr. Andrew Willet is one of the founders of SouthSide Family Practice in 1995. He went to Franklin and Marshall College, then Jefferson Medical College. After a family practice residency in Wilkes Barre, Pennsylvania, he worked in a community health center near Waynesburg, PA. He moved to Dover in 1989, and was chairman of the department of family medicine, as well as president of the medical staff at Kent General Hospital. He is married and has one son.
Dr. Brian Horn graduated from the University of Medicine and Dentistry of New Jersey, School of Osteopathic Medicine in 1997. He completed his residency program through the John F. Kennedy Memorial Health system in Stratford NJ and Our Lady of Lourdes Medical Center in Camden NJ in 2000. Upon competion of the program he joined Dr. Andrew Willet at Southside Family Practice in Dover where he has continued to pratice. Dr Horn is board certified in family practice and treats patients of all ages at both the Dover and Smyrna offices. He currently lives in Smyrna and has two children.
Allison Ford graduated from Clemson University in 2005 with a bachelors in Biological Sciences. She then attended Arcadia University where she earned her masters in Medical Sciences in 2007 and became certified to practice as a Physician Assistant. Soon after graduating she joined Southside Family Practice where she has continued to practice. Allison treats patients of all ages and is currently accepting new patients. Since joining Southside, Allison has gotten married and gave birth to a beautiful daughter, Makenzie.
