Southern New Hampshire Health has transitioned from offering the eCare patient portals to SolutionHealth MyChart. This transition is part of a substantial upgrade to our medical records system to Epic. SolutionHealth MyChart consolidates your medical history into one record available across Southern New Hampshire Health and Elliot Health System.

How do I Reset my primary care portal password?

Contact your primary care providers office. Forgot your password? Use the ‘Forgot Your Password’ link on the login screen to create a new one. Other issues? To reset a locked account, or if you are experiencing an issue connecting to your portal, call (855) 211.9499.
