What can I do in the patient portal?

The Southern Maine Medical Center (SMMC) Sleep Lab has moved to the Goodall Health Center located at 2 Livewell Drive in Kennebunk, into the suite previously occupied by Goodall Hospital’s Sleep Institute. Beginning on June 11, all sleep studies for Goodall and SMMC patients will be performed at our new location.

The Health Information Management Department is dedicated to maintaining your medical records and keeping your health information private and secure in accordance with patient’s rights and federal and state regulations.

How to get a copy of my health records?

You may request a copy of your records on paper, on USB flash drive, or on a compact disc (CD). A completed and signed Authorization to Release Protected Health Information form along with valid identification is required for copies of records to be released. To request the form be faxed or mailed to you, please call 207-662-2211.
