Advance Directives

If you have a Living Will, please bring a copy with you to every hospital inpatient and outpatient visit. If you do not have a Living Will, but would like one, please speak with the Admitting Office upon registration.

Insurance Information

Please bring your insurance card when you are admitted. We will make a copy of your card so we can bill your insurer directly for your covered benefits. Your co-payments are expected at this time. It is important that you understand what your insurance covers and where you can go for care.

Outpatient Testing

Make sure your doctor has called the hospital to order your procedure. Or, if the doctor gives the order to you, be sure to bring it with you when you report for testing.

These guidelines will help you prepare for your inpatient or outpatient surgery.

Keep track of your prescriptions and set reminders to take medications and order refills by going to MyMedSchedule.

What to bring to a syringe appointment?

To avoid delays at check-in, please bring the following with you on the day of your appointment: 1 Your picture ID or driver's license 2 All insurance cards 3 Prescription for services from your physician's office 4 List of current medications and allergies 5 When appropriate, please bring with you copies of any medical notes, lab results, biopsies, X-ray results, etc. 6 Your authorization or referral from your physician's office 7 If you arrive without your necessary authorization and/or referral, you will be expected to pay for your visit prior to receiving services or opt to reschedule your appointment. 8 Anticipate a call from the pre-registration area to collect your required estimated financial responsibility. Unpaid co-payments and deductibles are expected to be paid at the time of your visit.

How long does it take to register for a hospital?

Once you have completed registration, you will be directed to the appropriate area of the hospital. This process typically takes 10 – 15 minutes if the information you bring with you is complete.
