This includes access to online medical records (patient portal). If you would like to set up an account for our MyHealthRecord patient portal, please send us a message or call (360) 456-1900 and choose option 1. We will send you an invitation to register.

COVID19 Vaccine for Children Ages 5 Years and Older

Yale New Haven Health (YNHH) and Stamford Hospital provide Pfizer’s COVID19 vaccine for children ages 5 years and up. Please see our COVID19 Resources page for additional guidance. We look forward to providing the COVID19 vaccine here in our office soon and will keep you informed!

We disinfect all high touch areas constantly and every room between each patient use. We track and segregate well and sick rooms.
