Somerville Family Practice has transitioned to EPIC, an electronic medical records system that allows us to easily coordinate your care with other medical institutions in the greater Boston area.
For Patients Who Are Minors: Due to federal and state confidentiality laws, patients who are between the ages of 14 and 17 are not required to disclose certain types of medical information to parents/legal guardians.
Parents/guardians may communicate with FPC providers and staff as you always have until your child reaches age 18.
For this reason, we find it necessary to ask our patients in this age group to complete a consent form naming a parent/guardian as their portal authorized representative. Without this consent from the minor patient, we will not be able to give parents/guardians access to the minor's portal. (Click the link below to access the form.)
