The release form is required to allow the transfer of medical information from Smith College Health Services to a secondary provider, such as your primary care provider at home or a referred provider.
Spring admission deadline is January 11. All new entering students must submit all information by the deadline. Undergraduate Athletes. If you are playing an NCAA team sport you must also submit the NCAA Pre-Participation Physical Exam (last page of the health form) signed by your medical provider by deadline.
Students and employees are required to participate in the college’s COVID-19 screening program. Students will be tested twice weekly until at least Friday, September 10. Employees will be tested weekly for those fully vaccinated and twice weekly for those who have an approved exemption from vaccination.
Any student in residence who tests positive for COVID-19 will be moved to dedicated isolation housing for care and monitoring. Meals, transportation to and from medical care, and extended cleaning protocols will be provided for students in isolation.
Employees returning to on-campus work and new hires must receive a negative COVID test from the campus testing site within the three business days prior to returning to campus.
