To get to your patient portal, you’ll need to contact your medical provider’s office for the web address. They can also provide you with instructions on how to register and log in if you need them. Ascension Network (Tier 1) providers are Ascension-employed or contracted providers that have partnered with Ascension to provide high quality healthcare to our members. Members receive the highest level of benefits and the lowest out-of-pocket costs when they choose an Ascension Network provider.
Ascension Network (Tier 1) facilities are Ascension Ministries or contracted facilities. By visiting a Tier 1 facility, members get the most bang for their buck and receive the highest level of benefits SmartHealth has to offer.
