Discharge planning will assist you with planning a safe, appropriate discharge either to home or to an alternative level of care, such as a skilled nursing facility.
If you or a patient requires an interpreter or has special speech or hearing impaired requirements, inform a nurse or other staff member for assistance. Resources are available to assist speech and hearing impaired patients.
