What is Agapé at home?

Agapé @ Home realizes that each client has unique care needs. We offer a full range of in-home services that are customized to provide our senior and other adult clients the right amount and types of quality assistance that is needed and deserved, while still promoting independence. Agapé @ Home offers Companionship, Socialization, Cognitive Stimulation, Light Housekeeping, Meal Time Supervision & Set Up and Transportation to a weekly Life Enrichment Visit which includes an activity with their peers and lunch at one of our very own Assisted Living Facilities. Each client will be accessed and a customized care plan will be tailored to fit their individualized need.

Can Agapé@Home administer medications?

Agapé@Home cannot administer medications to the clients; we can ensure that our clients take their medication as prescribed by their doctor. We can assist with opening medication containers, reading labels, and reminding the client when it is time to take their medications.

Registered Nurse - Hospice House

The Registered Nurse is an integral part of the hospice team and as such is responsible for carrying out nursing duties to Hospice House patients and families in accordance with the patient's plan of care. He/she will plan, implement and evaluate nursing care through systematic assessment of health needs and optimum utilization of personnel.

Duties

Provides and manages nursing care to assigned patients/families and emotional support to patients, families and caregivers. Performs initial assessment of patient/family for admission to Hospice as assigned. Performs admission to Hospice House as assigned, including orientation of patient/family to rules, regulations and services of Hospice House. Performs daily nursing assessment. Works with other disciplines to provide quality patient care. Provides service to patients/families based on Plan of Care. Documents care given daily following Hospice guidelines for documentation. Teaches families and caregivers appropriate technique for providing care for patients. Monitors equipment and supply needs. Actively participates in interdisciplinary team meetings, case conferences and other planning meetings and works effectively in the IDT environment. Participates in staff development activities and in-service education. Assists in the development and revision of the Plan of Care. Available for day, night, weekend and holiday duty as assigned. Maintain confidentiality and professional conduct at all times. Demonstrates a positive attitude and working relationship with staff members and others. Carries out position responsibilities in a manner that exemplifies excellent customer service. Consistently represents Hospice House to all internal and external customers with integrity and professionalism. Provides a safe environment for patients, employees and visitors. Adheres to all policies, procedures and regulations regarding patient care, conduct, safety, fire security, confidentiality and risk management. Carries out all other duties as necessary..

Qualifications

Must have a current RN license in South Carolina Certification in hospcie and palliative care preferred (CHPN). At least two years of nursing experience preferred, current knowledge of nursing practice, experience in geriatrics and end-of-life care preferred Ability to maintain confidentiality and professional conduct at all times. Excellent verbal and written communication skills, problem solving skills, logic and reasoning skills. Some risk of exposure to communicable disease. Understanding of and committment to the hospice philosophy and its concept of care. Ability to perform duties under pressure. Ability to bend and stoop on a regular basis.
