How do I log in to my Patient Portal?

To get to your patient portal, you’ll need to contact your medical provider’s office for the web address. They can also provide you with instructions on how to register and log in if you need them. How do I access my health records online?

Access and view your health records at any time through our secure patient portal through athenahealth. You will find information for past visits, lab results, and other medical reports. If you are new to CHAS Health or don't already have an athenahealth account, sign up today for online access to your health records, labs, test results, and more.

How do I connect to Siu medicine as a connected organization?

If you are a Springfield Clinic or Memorial Physicians Services patient portal user, you can use the same portal account and add SIU Medicine as a connected organization.

How will my patient access their test results?

All patients will have immediate access to test results and provider notes through their patient portal, whether they are in a hospital, urgent care center or provider practice. Genetic testing results are not subject to these requirements.

New! Expanded Access to Your Medical Information

The 21st Century Cures Act is a recent federal mandate that expands access to medical information. Pursuant to the Cures Act, Nuvance Health will release most test results (excluding genetics) and provider notes through the patient portal as soon as they are available.

If you’re not already signed up for an SIU patient portal and are 18 years of age or older, you can either stop by your provider's office or request a new account . If you are a Springfield Clinic or Memorial Physicians Services patient portal user, you can use the same portal account and add SIU Medicine as a connected organization.

Manage your family's health

The proxy feature gives you access to the records of your minor children, birth through age 11, and dependent adults or adults for which you may have legal guardianship, power of attorney, etc. SIU Medicine must have any legal documents on file to allow proxy account access. Request a proxy account at the patient's SIU Medicine provider's office.

Teenage Children

SIU is committed to the privacy of its patients. In the state of Illinois, anyone over the age of 12 has protected minor status, which means they have the right to privacy, even as a dependent. For this reason, SIU does not provide online portal accounts for children between the ages of 12-17.
