Your medical information is easily accessible with My Chart. Review your immunizations, allergies, summaries of recent visits to the doctor and current health concerns whether it’s on your desktop, your laptop or your phone.
Making an appointment to see your provider is as easy as logging on to My Chart. During times that work with your busy schedule, view your provider’s availability and select the appointment that works best for you. When something unexpected happens, My Chart allows you to cancel appointments too.
My Chart lets you conveniently request prescription renewals for you and your family.
When you have a non-emergency question or health concern, you can email your provider directly and securely with My Chart. You care team can email you as well. It’s a great way to exchange information without having to coordinate schedules. Responses are typically within two business days.
With My Chart your doctor will send normal test results as soon as they’re available. You can also see any provider notes and comments so you can better understand those results.
My Chart allows you access to medical bills online and submit payments.

On June 1, 2014, Sidney Health Center converted to a new electronic health record (EHR), offering a new opportunity for patients to access their personal medical information via a secure website that is managed by Sanford Health. MyChart is a patient portal that allows patients to participate in their care by accessing a summary of medical information.
Why should patients sign-up for this service? First, patients benefit from the convenient access MyChart provides. In addition, adults can sign-up their children or spouses, and have quick-access to vital information regarding recent labs, upcoming appointments, or overdue health maintenance.
To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.
All messages sent to your provider’s office are transferred via a secure server compliant with HIPAA standards.
The RWJBarnabas Health Patient Portal is a free service that provides convenient access to your personal health care information in a secure online environment. With the patient portal, you can view select test results, access your medical records and track your health information.
At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records. Due to the sensitive nature of radiology and pathology test results, these are not available on the patient portal and must be obtained through the hospital’s Health Information Department.
RWJBarnabas Health is in the process transitioning all of our hospitals to a centralized patient portal called MyChart where all hospitals’ records will eventually be contained within one system for your convenience. Please be assured that you will always have access to your medical records.
