If you need to obtain a copy of your or your child's medical record you can submit a request for release to Shriners Children's at any time by utilizing one of the following methods:

Shriners Children's outpatient clinics are where you and your child will have your initial consultation and follow-up visits with our specialists. If you are bringing your child in to be assessed by members of our care team, we will schedule your initial appointment at our outpatient clinic.
If your child is accepted for care at Shriners Children's, you will receive a condition of acceptance form along with your initial appointment letter. The form must be signed by the patient's parent or guardian, witnessed by someone age 18 or older. By signing this form you are giving Shriners Children's permission to treat your child.
Applicable legal documents: Please bring any legal documents pertaining to custody, divorce, separation, adoption or name change of a parent or child.
