What is a medical record?

Your medical records are a collection of documents that represent your visits and treatment at Shore Medical Center. Health information management services staff are trained to ensure that this information is complete, confidential and accessible to you. The privacy and confidentiality of your records is protected by law.

How long does it take for a health information request to be processed in New Jersey?

Health Information Management Services (HIMS) makes every effort to process requests expeditiously. The New Jersey state regulation allows up to 30 days from the date of the request to comply with any request to release information.

What is the purpose of a medical authorization?

Purpose for which the information may be disclosed (i.e. person, attorney, physician) To whom the information is to be sent (name and address) The patient's signature or a patient's legal representative's signature. Authorizations signed by a representative must be verified.

Does Shore Medical Center discriminate?

Is Shore Medical Center tobacco free?

When will New Jersey Hospital be green?

How old do you have to be to visit a hospital?

Upon entering the hospital, you will present your ID and undergo a symptom and temperature check. All visitors must be 18 years of age or greater, with rare exceptions. You will receive specific guidelines for visitation from the front desk staff.

Test Results

Your test results will be made available to you through the patient portal soon after they are finalized, and likely before your provider has had a chance to review them. It can be difficult to understand the meaning of test results without the input of your provider.

New Feature: Patients Can Send Attachments To Secure Messages Sent Through The Portal From Their Desktop Computer and Mobile Phone

You Can Now Check in for Shore Physicians Group Primary & Specialty Care through the portal

Two days prior to your scheduled appointment, you will receive an email notification that it is time to check in for your appointment. We encourage you to stop at the front desk and update your communication preferences to ensure you are able to check in online.
