How do I contact my shore portal?

Shore Clinical Portal To obtain access to your patients' medical records through the Shore Clinical Portal, please email to remoteaccess@shoremedicalcenter.org . From there a representative will work with you on the provisioning and training process. Our goal is to respond to your email within three business days.

Test Results

Your test results will be made available to you through the patient portal soon after they are finalized, and likely before your provider has had a chance to review them. It can be difficult to understand the meaning of test results without the input of your provider.

New Feature: Patients Can Send Attachments To Secure Messages Sent Through The Portal From Their Desktop Computer and Mobile Phone

You Can Now Check in for Shore Physicians Group Primary & Specialty Care through the portal

Two days prior to your scheduled appointment, you will receive an email notification that it is time to check in for your appointment. We encourage you to stop at the front desk and update your communication preferences to ensure you are able to check in online.

What is South Shore MyChart?

This patient portal is called South Shore MyChart, it is available to all South Shore Health patients .

What is a pediatric proxy?

Pediatric and Disabled Teen proxy access allows you to securely communicate on behalf of a pediatric patient who is under the age of 13 (or 13 -17 years if the child is disabled). Access is obtained by completing the Pediatric and Disabled Teen Proxy Authorization Form. When the patient turns 13, this will transition to “Adolescent Proxy” access, with the ability to view a limited portion of the patient’s records.

What is a proxy access?

Proxy access allows a parent, guardian, or other designated individual to connect to the South Shore MyChart account of a family member or friend. Access is granted once a signed paper authorization form has been completed and processed by the patient’s primary care site location.
