Request a hard copy of your medical records

We protect your health care information in compliance with federal and state requirements. If you’d like a copy of your medical records, please download the Release of Information form for your hospital listed below, then print, complete, sign and mail (or fax) the form to the hospital.

How to access my health records?

With My Health Rec, you can: 1 Privately view some pathology, radiology and lab results online. 2 Access health records such as immunizations, medications and allergies 3 View hospital records such as discharge summaries and notes about procedures, progress and consultations (including wound care, PT/OT/SLP, dietary, etc.) 4 Download health records (history and physical) to easily share with doctors 5 Save time by paying health bills online

My Health Rec is a great way to view, download and transmit your up-to-date health information, all from the convenience and privacy of your own home, or anywhere Internet access is available — 24 hours a day, seven days a week. We recently added opportunities for additional documents you can see online, such as pathology reports, discharge summaries and progress/procedure notes.

Comprehensive Diagnostic Imaging for Central Alabama

Fast and convenient diagnostic imaging—from MRIs to CT scans—are a vital step in diagnosing many medical conditions. Diagnostic imaging may include the following services:

Hospital Outpatient Diagnostic Centers (HOPDs)

HOPDs are usually run by a radiology group or hospital and perform testing for in-house providers. They also perform physician services, such as diagnostic test reading at the location where the diagnostic tests are performed.

Independent Diagnostic Testing Facilities (IDTFs)

IDTFs are independently owned and furnish test results to outside providers, such as a primary care physician or other referring physicians.

What is Baptist Connect?

The My Baptist Connect website is a safe, private way for Baptist Health patients to stay connected to their personal health information.

How to enroll in my baptism?

To enroll, all you need is a secure Personal Information Number , or PIN. We’ll send you a secure PIN by mail after your visit — or we can email it to you as a part of your registration process, if you prefer. Once you receive your PIN, use it to complete your registration at MyBaptistConnect.com – the secure link will be listed in your instructions.

How to access my child's medical records?

View your child’s medical records 1 Parents and legal guardians can request access to their child’s (newborn –11 years old) medical information. 2 Adolescents 12-17 years may have their own portal account. 3 Request access to the portal from your Baptist pediatric provider or visit any Baptist Health hospital registration desk. 4 You will be asked to show photo identification as the child’s parent or legal guardian and sign an authorization form. If the legal guardian is not yet established in the patient registration system, you will be asked to visit the Health Information Management (HIM) department at any Baptist Health hospital to show the guardianship court order.

How old do you have to be to have a portal account?

Adolescents 12-17 years may have their own portal account.

Can an 18 year old access another adult's portal?

Adults 18+ may grant access to their portal account to another adult.
