What is the patient portal?

SHaW will be providing COVID-19 vaccines by appointment to our Storrs based graduate and undergraduate students when they become available to us. Plan ahead for preventive medical services. If you are in need of preventive care (such as a physical exam or certain screenings) please contact our Medical Care scheduler by calling 860-486-2719.

If you choose to use the Patient Portal, your name and e‐mail address will be treated with the same care and privacy given to your medical records.

Who can sign up?

Patients and their legal guardians can sign up for Patient Portal in accordance to the Terms and Conditions of the website. Just ask your doctor about enrolling and you are on your way to connecting with your healthcare team.

How can you sign up?

You can sign up for the Patient Portal by providing your name and email address to the front desk representatives. You will be instructed to create your own private password.

Important Information for Parents

Shawnee Health Care has a legal responsibility to protect the privacy of a minor (age 12-17) who requests certain services. Illinois law allows a minor to confidentially seek care for services such as: birth control, sexually transmitted diseases, HIV testing, substance abuse, and mental health services.

When do I need to apply for allied health stipend 2021?

Please complete the allied health stipend and submit by June 11, 2021. You must be a graduating senior from Cleveland Central High School, McEvans School, or Northside High School. Applications can be submitted via email to shawfamilymedica@gmail.com
