How do I release protected health information to the patient?

Who can request medical records?

Medical records may be requested by a competent patient if the patient is 18 years of age or has been emancipated. Medical records of a deceased patient may be requested by the personal representative of the patient's estate. If the deceased does not have a personal representative, his or her spouse may make a request.

Can a patient inspect medical records?

Releasing Medical Records. A patient (or his/her legal representative) may inspect and/or obtain a copy, or have copies of medical records sent to another facility. There may be a charge for this service.
