How to enroll in my seton health portal?

To enroll in the My Seton Health portal, which contains your hospital care information, please fill out the appropriate form below and bring it, along with a government-issued ID, to any Ascension Seton or Dell Children’s location. You will then receive an email to complete your registration.

How long does it take to get a copy of your medical records?

Please allow two weeks to receive a copy of your requested medical records; please note that some records are not complete until 30 days after discharge as they are awaiting transcribed reports from healthcare providers.
