At Ascension Seton and Dell Children’s Medical Center, we make accessing you or your child’s health information convenient with our patient portal. You can view information from your visit at Ascension Seton or Dell Children’s Medical Center through our patient portal. There are separate portals for hospital visits and medical office visits.
To enroll in the My Seton Health portal, which contains your hospital care information, please fill out the appropriate form below and bring it, along with a government-issued ID, to any Ascension Seton or Dell Children’s location. You will then receive an email to complete your registration.
