How do I schedule a telehealth visit? 1 Medical - Contact your local medical clinic from the list at the top of this page. If you have a MyChart account, schedule your telehealth visit online through the MyChart portal or mobile app. 2 Behavioral Health - Contact your local behavioral health clinic. Find contact information of your local clinic from the list at the top of this page. 3 Dental - Contact your local dental clinic. Find contact information of your local clinic from the list at the top of this page. 4 Just as with an in-person visit, please remember to cancel your appointment by calling your clinic if you are no longer able to make the appointment time.

What to call 911 for?

Call 911 for medical emergencies. Telehealth is not for emergencies or potentially life-threatening medical conditions such as chest pain, severe shortness of breath, severe abdominal pain or headache. If you experience these symptoms call 911.

Can I use Zoom on my tablet?

Yes, if you are using a smartphone or tablet for your virtual visit, you will need to download the Zoom app. Download the app in App Store or Google Play. On the day of your virtual visit, you can join your zoom meeting by opening the Zoom app and entering your meeting ID.

What is a "proxy" access for a health record?

Proxy access allows a guardian or other legally authorized adult to log into Sea Mar's Online Health Record via FollowMyHealth and connect to the account of a family member or another person. Proxy access is provided when the patient is unable to create their own portal account or at the request of the adult patient.

What paperwork do I need to have?

In most instances, you will need to provide legal documentation to establish your right to act as the personal representative of the adult patient. Examples of legal documentation include legal guardianship and durable power of attorney for healthcare. Please provide copies to one of our Sea Mar medical clinics.

When will I be given access?

For the protection of our patients, proxy accounts will be granted to people who have the legal right to request medical information on behalf of our patient. Because this must be verified through our Health Information Management Department, proxy accounts may take longer to complete.

How do I sign up?

To request a proxy account, please fill out the proxy form with the requested level of access. Please drop off the completed form in person at our one of our Sea Mar medical clinic and bring proof of identification. When the document has been verified, you will receive a proxy account invitation via the email address you provide on the form.
