Click to Create An Acct Patient Portal.pdf now - You cannot enroll/set up an account until the admissions/registration staff at SCH or our clinic staff have entered your email address into the new system and given you your medical records number.
How do I link my GP practice to patient access?
Once you have clicked on Verify my email address in the verification email that was sent to you, you can then click Sign in to Patient Access. Link your GP practice (to link to your practice).
How do I verify my Patient Access Account?
The account is created. Verify your email address. Now your account has been created you must verify your email address. An email will be automatically sent to the email address you registered your Patient Access account with. This will provide you with a link to verify your account.
