Using this portal, you will be able to access your health information online. All you need to activate this services is a valid email address. You will then be able to access information from your visit, allergies, procedures, lab and radiology results, vitals and more - all in one place.
Please contact our Portal Help during Regular Business Hours, Monday – Friday, 8:00 am – 4:30 pm. They can assist you in accessing your Patient Portal Account. To start the process outside of a visit, complete this Non-Patient Portal Consent Form, and along with a copy of your driver’s license, email to Portalhelp@spartahospital.com, or mail to Sparta Community Hospital, Attn: HIM, P.O. Box 297, Sparta, IL 62286.
If you are given consent by the patient, you can be set up as a portal authorized representative. Start by completing this Authorization for Authorized Representative Access to Patient Portal. It requires the patient and representatives signature. Email the signed form to the portal help desk or mail to the hospital address above.
