How do I contact Sanford Health Plan's coverage concierges?

In order to process your request, the patient must sign the Adult Proxy Form and provide authorization for release of medical information in My Sanford Chart on the page titled "Adult Proxy Authorization for Release of Medical Information". Please note that the patient's chart will be accessed through your (the proxy's) My Sanford Chart record.
