Email both items to PortalEnrollment@salemregional.com. Please include your 'Enrollment Email' in the body of your email message. You will receive a return email within 1-2 business days about the status of your Portal enrollment, along with a temporary logon and password, which is needed to set up your account.
If you provided your email to SRMC staff before 1/12/21 and have not yet enrolled in the Patient Portal: Using the email address you provided to SRMC staff, complete the online "Self Enrollment" page by clicking here.
