To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.

How to log into Portal?

To log into the Portal, click the bar at the top of this page. Then, simply enter your user name and password.

Can you copy and paste your password on the Portal?

Yes. Portal passwords are encrypted and URLs are re-written so that they cannot be copied and pasted. You and authorized family members are the only ones who can access your Portal. Also, a timeout feature protects your information if you leave the Portal page open.
