How To Request Medical and Imaging Records

Rothman Orthopaedics has partnered with MRO eXpress for the completion of medical and imaging records.

Proof of Identification

All requesters must provide proof of identification and/or documented proof of legal relationship, as appropriate, along with the request form.

Processing Time

Requests are processed in the order they are received and will be delivered to you as quickly as possible.

Fees for Records

Depending on the purpose of your request, you may incur a fee for a copy of the records. You will be advised of any potential fees when your request is submitted and again before it is completed.
