In order to obtain copies of your records or to authorize release of your medical records to another party, you must complete, sign and date an Authorization to Use and Disclose Protected Health Information form. To hand-deliver this form, visit any Rockwood Clinic location, Monday to Friday, 8am-5pm, or call 509-838-2531.
