OPTION 2 – To make an appointment, leave a message for your physician or to follow up for a prescription you already requested through your pharmacy.

Extended Service

Please note there is an extended service code that will be reported to your insurance company for appointments before 8:00 am and after 6:00 pm. In addition, a holiday service will be reported to your insurance company for appointments that fall on Martin Luther King Day, Columbus Day and Veterans Day.
