Clinical Visit Notes Now Available for Viewing in your Patient Portal

Effective March 17th, 2021, RAWNY is pleased of offer full access to clinical visit notes from your appointment, that are available for viewing/download via your Medent Patient Portal. Please allow 1-2 business days after your visit for your notes to appear in your portal.

Medical Record Amendment Request

As a patient, you have a right to request an amendment (correction/changes) to your medical record if you believe the information is incorrect or incomplete. In order to make this request, you will need to submit a completed Amendment Request form.

After your visit with our physicians, you will able to access to the summary of your visit on our patient portal. Upon checking out at our facility, please request instructions on how to access the portal, along with a passcode that will allow you to view your medical record.

Is a mask required for Retina Associates?

Updated COVID policy in effect at all Retina Associates of Cleveland offices: Masks are welcome but no longer required for those who have been vaccinated, per new CDC guidelines and effective May 20th, 2021. Family members are once again welcome to accompany patients to their appointment if needed.

Michael Borne, M.D

Dr. Michael Borne received a Bachelor of Science degree in Pharmacy from the University of Mississippi. He graduated Cum Laude from the University of Mississippi Medical center and completed internship training in Birmingham, Alabama.

Jay Brown, M.D

Dr. Jay Brown received his Bachelor of Arts degree in Biology from the University of Mississippi, and he completed his medical degree and Ophthalmology residency at the University of Mississippi School of Medicine in Jackson, Mississippi.
