The typical starting dosage for adults, ages 18 to 65 years, is 18 mg to 36 mg taken by mouth once daily in the morning. Your doctor may increase your Concerta dose by 18 mg daily each week after starting the drug.
The Concerta tablet does not dissolve completely in the body after all the medicine has been released. You or your child may sometimes notice the empty tablet in a bowel movement. This is normal. Children should have their height and weight checked often while taking this medicine.
A headache, abdominal pain, a loss of appetite, dry mouth, a fast heartbeat, nausea, nervousness, and insomnia are commonly reported side effects. Rash, pruritus, irritability, an increase in blood pressure or excessive sweating may also occur.
Nervousness, trouble sleeping, loss of appetite, weight loss, dizziness, nausea, vomiting, or headache may occur. If any of these effects persist or worsen, tell your doctor or pharmacist promptly. An empty tablet shell may appear in your stool.
Mean times to reach peak plasma concentrations across all doses of CONCERTA® occurred between 6 to 10 hours.

Concerta did nothing to help, it'd wear off within 3 hours or less after taking it, which isn't very helpful when I'm needing to focus in class. It made me extremely moody, I couldn't show emotion, it constantly made my mouth very dry, and I gained a lot of weight on it.

When Concerta is taken, a shell on the outside of the capsule dissolves, releasing 22% of the total dose of the medication. Three inner compartments then release at different intervals as the chambers absorb fluid from the intestine; this results in extended medication release. The release, however, is not consistent. Concerta was the first “backloaded” delivery system (other backloaded systems are Metadate CD and Biphentin), which was designed to produce constantly increasing blood levels of methylphenidate over a period of about 10 hours followed by a rapid decline in blood level once the hard caplet has released all that it is going to release. One-sixth of the dose is never released so that an 18 mg caplet of Concerta really delivers just 15 mg and a 36 mg caplet just releases 30 mg, and so on. Even after 20 years on the market there is a disagreement among clinicians about whether a steady release delivery system or a backloaded, ever-increasing dose is better.

This means that Concerta is regulated by the federal government. It’s regulated because people can sometimes become dependent on the medication. Dependence on a drug means that your body needs to have the drug in order for you to feel normal. This condition can lead people to misuse a drug.
Concerta works by increasing levels in your brain of two neurotransmitters (chemical messengers) called norepinephrine and dopamine. Increasing these neurotransmitter levels helps to stimulate your brain, which can improve the symptoms of ADHD.

If you’re already taking another form of methylphenidate (the active drug in Concerta) and you’re switching to Concerta, your starting dosage of Concerta may be different from the typical starting dosage. Other drugs that contain methylphenidate include Quillichew and Ritalin.

I haven't taken Concerta, but I am on Adderall. I relate to your struggles and wanted to share my experience after I was finally diagnosed and treated. It was a complete life changer! In my non- expert opinion, I think the dose is fine. Concerta is supposed to be much more mild than most of ADD stimulants out there.
