If you are living with congestive heart failure, it’s important to monitor changes in your condition. Rochester Regional Health’s eHealth at Home program enables us to manage your health from the comfort of your own home.

How it Works

Using a combination of technology and home visits, your care team will track information related to changes in your health. If we find that your treatment needs adjusting, we’ll contact you and take immediate steps to modify your care. In many cases, you can be treated in your home for medical conditions that traditionally require a hospital stay.

Visiting Hours

Although we are staffed 24/7, visiting hours are daily from 8am – 8pm. Please follow the latest Rochester Regional Health guidelines for visitation, as there are some restrictions due to the current pandemic.

Your Arrival and Admission

About Your Stay

Each patient will be assigned a private room, which includes a bed, entertainment area, and bathroom. Our staff will show you how to use the parts of your room and you’ll have a call button if you need help at any time. Meals are provided via a pre-selected menu featuring a special each meal.

Discharge

Our team will work with you and your family to make sure all of your discharge planning needs are met whether it’s to home or to another facility. We want to assure you will receive the necessary follow-up and that your discharge is safe.
